The Ministry of Health and Social Development has launched a National Public Health Symptom Reporting Form (SRF) to better reach and manage persons in the community who are experiencing symptoms of COVID-19.
According to Chief Medical Officer Dr. Ronald Georges, the reporting process starts with the completion of the initial intake form. Once submitted the Public Health Officers will review and liaise with the appropriate medical centre to arrange for treatment and/or testing.
The applicant will then be sent a follow-up monitoring form at the same time daily to update until they have recovered. All submitted forms will be reviewed by a health team.
“We want to encourage all persons in the community to utilise this reporting tool if they are experiencing any COVID symptoms,” Dr. Georges confirmed.
Symptoms include but may not be limited to:
1, Fever >100.4F (38C)
2, Subjective fever (felt feverish)
3, Chills
4, Muscle aches (myalgia) (e.g. low back pain)
5, Joint Pain
6, Runny nose (rhinorrhea)
7, Sore throat
8, Cough (new onset or worsening of chronic cough)
9, Shortness of breath (dyspnea)
10, Nausea
16, Vomiting
11, Headache
12, Abdominal pain
13, Diarrhea (≥3 loose/looser than normal stools/24hr period)
14, Confusion/ Irritability
15, Swollen toes
16, Coughing up blood or bloody mucous
17, Weakness, tingling or numbness in feet and hands
