Employers are notified that the following information should be included when submitting their assessment of their business in light of the COVID-19 pandemic:

  • The company’s name.
  • The status of the business.
  • The names of their employees.
  • The positions of their employees.
  • Indication of whether they are full-time or part-time.
  • Any payment packages being offered.
  • The location of the employees.
  • Any comments that they deem to be useful.
